In accordance with the City of Hialeah Code Section 2.817 ( e) (1) Exceptions to bid: In addition to the monetary
threshold provided in section 2-812, competitive bidding is exempted for professional services in the nature of
consultants, accountants and attorneys, sole source providers; emergencies based on objective facts; and credit
facilities.
I am respectfully requesting approval to use Sole Source Vendor and open a Purchase Order in the amount of
$15,000 for REV/RTC, (DBA Hall-Mark RTC), (V.N.#16195), 3431 N.W. 27th Avenue, Ocala, FL
34475. This purchase order is for repairs of out of service apparatus with E-One only authorized
repairs & service dealer in the State of Florida (Sole Source). This is an "Active Corporation" in good
standing in the State of Florida. Your approval will allow us to obtain factory authorized service & repairs
on our E-One fleet of fire trucks.
Funding for this expenditure will be drawn against Account #109.2000.522.463. (Repair & Maint. -Vehicles)
